Formation
The rule
How to form it: take the ellos form of the Pretérito indefinido and drop -ron: hablaron → habla-, comieron → comie-. Add the endings -ra, -ras, -ra, -ramos, -rais, -ran.
Accent: the nosotros form always has a written accent on the syllable before the ending: habláramos, comiéramos, viviéramos, fuéramos.

The -ra and -se forms
The -ra form
-ra, -ras, -ra, -ramos, -rais, -ran
More common in speech, especially in Latin America. Only this form can replace the conditional in a main clause: quisiera, debiera, hubiera.
The -se form
-se, -ses, -se, -semos, -seis, -sen
Equivalent to -ra in subordinate clauses: hablara = hablase, tuviera = tuviese. Heard in Spain and found in writing.
Tip
Learn both
You can get by speaking with -ra only — everyone will understand you. But you need to recognise -se: it is everywhere in books, newspapers and in the speech of people from Spain.
Irregulars
Where the irregularity comes from
There are no separate exceptions: all the irregularity comes from the Indefinido. If you know tuvieron, dijeron, fueron, you know tuviera, dijera, fuera.
Watch out: dijeron → dijera (not “dijiera”), trajeron → trajera, leyeron → leyera. Ser and ir share one form: fuera — the context tells you which.

The rule
Sequence of tenses. If the main verb is in a past tense or the conditional and calls for the subjunctive, the clause takes the Imperfecto de subjuntivo: Quiero que vengas → Quería que vinieras; Me gustaría que vinieras.

A conditional in the main clause
A conditional in the main clause also calls for the Imperfecto de subjuntivo: Me gustaría que vinieras, Sería mejor que te quedaras, Preferiría que no dijeras nada.
When the present stays
The Pretérito perfecto in the main clause is usually linked to the present, so it is more often followed by the presente de subjuntivo: Le he pedido que venga mañana.
If a past request still holds, you may hear the present in everyday speech: Me pidió que lo llame mañana. The safe choice is the Imperfecto: Me pidió que lo llamara.

Como si is always followed by the Imperfecto (or Pluscuamperfecto) de subjuntivo, whatever the tense: Habla como si lo supiera todo.

A wish that is unlikely to come true or is unreal right now: Ojalá estuvieras aquí.

How to remember
Como si never takes the present: “as if” is always imagined, and in Spanish the imagined goes into the past subjunctive.
